For Providers Referring a Patient

  1. Send us a secure message with the referral details:

  2. Or Fax us at 470-826-2233

  3. Include:

    • Patient’s name, DOB, and contact information

    • Reason for referral / clinical concerns

    • Any relevant documentation (intake notes, labs, med list)

  4. We’ll take it from there! Our admin team will contact the patient directly and confirm next steps.
     

For Patients (Self-Referral or from Provider)
